Identifying models of care for palliative care in residential aged care

Background: As Australia’s population continues to age, residential aged care facilities (RACF) are becoming an increasingly key part of our health system. Almost one in three Australian deaths occur in a RACF. This means that the provision of palliative care should be core business for RACF. However, the recent Royal Commission into Aged Care Quality and Safety found that end-of-life care in RACF was often inadequate. The Commission described palliative care as an area for immediate attention. We are exploring what models of care exist to provide end-of-life care in a RACF setting. Understanding the existing models and their strengths and limitations is needed to drive healthcare reforms for appropriate and timely palliative care provision to the residential aged care sector.

Aim: To identify the existing models of care for the provision of palliative care in RACF.

Methods: Ovid MEDLINE, PsychInfo, and CINAHL Plus were searched to identify the relevant studies. The population group was defined as residential aged care residents aged 65 years and older. Studies describing or evaluating a model of care for palliative care delivery to this population were included. Results were limited to publications in the English language and from 2009 onwards. No restrictions were made by country.

Results: The initial search identified 2225 studies. After the removal of duplicates (836), abstract screening and full-text review there were 31 studies included. These were from 9 countries covering the domains of development, implementation, and evaluation of models of care in the provision of palliative care in RACF. There were also findings on the impact of the COVID-19 pandemic. Further findings will be presented as the analysis is ongoing.

Conclusions: Models of care provide implementation lessons to ensure appropriate and timely provision of end-of-life care to RACF residents and make improvements to the residential aged care sector.
