Institution: THS Specialist Palliative Care Service, NW Tasmania. - Tasmania, Australia
THS Specialist Palliative Care Service introduced Comprehensive Palliative Care in Aged Care as an in reach service to Residential Aged Care Facilities across Tasmania as part of a three year Project funded jointly by the State and Federal Governments to improve Palliative Care and End of Life Care for older Australians living in Residential Aged Care.
In the Project's first year over 600 residents have accessed the service in 63 RACFs across Tasmania.
Tom and Dana work in partnership to cover the NW Tasmania Region which encompasses a total land area of 23,000 square kilometres, including King Island.
Using a Needs Round Model of Care they visit each RACF monthly to support residents ,their 'families', staff and GPs, to access and provide improved Palliative and End of Life Care, reduced hospital transfers and an increase in the choice of preferred place of death.
