Institution: Joint initiative between Opal Health Care and Palliative Aged Care Outcomes Program (PACOP) - NSW, Australia

Background
Although a Palliative Care approach existed which included an overarching Palliative Care Framework, there , there were no specific clinical palliative care framework and assessment tools available for clinical team to utilise to support clinical decision making.
 
Aim
·         To improve clinical confidence in identifying the dying process
·         Support discussion with families and multidisciplinary team
·         Improve care planning and availability of end-of-life medications
·         Support clinical team ability to facilitate advanced care directives with residents and their representative, from the point of admission and ongoing.
·         Enable a supportive dying process or ‘dignified’ death as per the resident’s wishes.
 
Methods:
·         Phased implementation approach (by region, by Care Community)
·         PACOP Improvement Facilitator supporting National Clinical Governance and Risk Manager in relation to policy review 
·         Training
·         PACOP Support Officer (0.4) FTE
·         Community of Practice monthly
·         Pall Centre for data collection 
·         PACOP Shop - resources
 
Results /findings
·         Team satisfaction with having a validated palliative care clinical assessment tools
·         Increasing awareness of frailty and its impact on health outcomes (clinical indicators)
·         Resident focused clinical assessment tools – supporting assessments to be conducted ‘with’ the resident not ‘on’ the resident 
·         Improved recognition of end of life with residents commencing an end of life care pathway more than 24 hours before their death, using ELDAC data for 2019 as a baseline for improvement
 
Lessons Learnt
·         Improved coordination and preparedness of end-of-life care provision
·         Support Care Communities with a system for 100% profile collection each three months as well as ensuring system for profile collection for new admissions between each quarter.
·         Education plan for each care community prior to first round of profile collection
·         Fairly ‘quick’ implementation for first round and difficult time of year: Education early December 2022 with 100% profile to commence in January 2023.
·         Schedule Profile collection months to NOT be the first month of the quarter
