Introduction
The Healthy End of Life Planning Brief Intervention (HELP BI) is a resource efficient clinical, social and digital intervention (HELP App) designed for the palliative care sector to support patients, carers and families to benefit from informal ‘network-cantered care’ as part of their treatment to sustain care and well-being in any setting. HELP BI is a brief intervention that uses evidence-based strategies to optimise an ideal palliative care journey where both formal and informal models of care seamlessly integrate. 

Background
HELP BI is the product of nine years of research and practice conducted by La Trobe University’s Public Health Palliative Care Program in partnership with palliative and other health services, community organisations, and people with lived experience of palliative and end-of-life care. HELP Digital is a recent program development aiming to centralise practice support, training, and data collection. 

Aims
This interactive workshop will draw upon the research findings and practice wisdom of an evidence-based health promotion approach to palliative and end of life care – HELP BI. The value, knowledge, practice and skills involved in facilitating a ‘network-centered’ approach to palliative care service delivery will be provided in this workshop. 

Key learning outcomes 
Participants will acquire:  
  • Knowledge of the evidence-base for social network enhancement strategies, brief interventions, and the principles of a ‘network-centered’ care approach to their professional practice 
  • Skills required to implement HELP BI with clients including needs analysis, evidence-based social network mapping interventions and implementation 
  • Process knowledge to facilitate HELP BI in both paper-based and digital form (HELP App) 
  • Deepened understanding of the enablers to harness social supports, and how to work with them effectively
  • Knowledge of the patient/carer journey where HELP BI can be implemented, and how various elements of the HELP BI Resource Package can be applied to suit context and time constraints.
